Length or Girth? What Really Matters to Women (and to You)

What-Really Matters-to-Women
Sam Margiotta, MD.

Research and surveys suggest that overall proportions and sensation, often influenced by girth, play a significant role in satisfaction, while length shows diminishing returns beyond a certain point.

It’s the age old question of ‘length vs. girth’. Which ‘dimension’ is more important to women?

My team and I decided to look into well-documented medical studies and surveys involving real women to see what they had to say. The results may surprise you.

Let’s Be Honest – You’ve Thought About it…

The question of length or girth has probably crossed your mind at some point. If you’re a driven guy in your 40s, 50s, or beyond, you’ve likely asked yourself:

“Do I need more length… or more girth?” “What do women want?” “Can I do anything to increase my dimensions?”

Maybe not out loud, and maybe not even seriously. After all, you have what God blessed you with and you’ll work with the tool you have, right?

But if you’re like most of our Boca Raton area patients, you’re curious.

And if you’re the kind of guy who works on improving things…your health, your energy, your business, then this falls into the same category:

What actually makes a difference? What do WOMEN prefer?

And CAN you actually improve what you were born with…the answer may surprise you.

Because there’s a big gap between what people think matters… and what actually does, we took a look at the recent medical and psychological studies to find out what women REALLY think.

What the Research Actually Says

One of the most well-known and highly referenced studies on ‘length vs. girth’ entitled “Penis size interacts with body shape and height to influence male attractiveness” (Mautz, 2013, p. 1) was published in the Proceedings of the National Academy of Sciences (PNAS) in 2013. This is widely considered the “gold standard” because it moved away from abstract numbers or 2D images to 3D modeling.

Research Actually

It directly examined female preferences by asking women to evaluate different male penis types, including variations in flaccid penis size, using 3D-generated images.

The findings showed that women did factor in penis size when rating attractiveness, but importantly, the relationship wasn’t linear, there were diminishing returns with length, while overall proportions and presence played a larger role in appeal.

This length vs girth study reinforces a key takeaway seen across multiple sources: balance matters more than extremes, and increases in girth tend to have a more noticeable impact than additional length alone.

girth tend

Another study from this year (2026), girth vs. length preference by PLOS Biology, which was featured in Men’s Health magazine,  also used 3D modeling to figure out what women actually notice when it comes to attraction.

The data was clear: proportions matter more than extremes. When it comes to male anatomy, the research shows that girth is a more significant factor for partner preference and satisfaction than length alone.

The takeaway is that the overall presence and “fullness” rather than just one dimension, but if one dimension needed to be selected, it would be ‘girth’.

A well-known survey published in The British Journal of Urology International reinforces this. This survey asked women about their ideal penis size for both long-term and short-term partners.

We also looked at several highly regarded medical and psychological surveys of women.

Across a range of surveys such as surveys published in journals like BJU International, clinical observations, and smaller preference studies over the past two decades, a consistent theme showed up: while size plays a role, most women don’t prioritize extremes as the most important thing, but when they do mention their preference, it’s often related to girth rather than length.

We’ve also found the same thing in our informal survey of our female patients on the other side of our practice – Jiva Med Spa…

Boca Raton women prefer girth over length at a proportion of about 4 to 1.

Why Girth Makes a Difference (In Real Life)

This isn’t just theory – it’s just mechanics.

Think about it like this…

Length is more about reach. Girth is about contact.

And contact is where sensation lives. A little more girth means:

  • More surface interaction with the vagina (or anus in homosexual sex)
  • More stimulation for both partners
  • A stronger sense of connection, not just physically, but psychologically

It’s subtle… but it’s real.

So Where Does That Leave YOU?

Most men aren’t looking to become someone else. They’re just looking to upgrade what they already have. And a little bit of a boost goes a long way.

You’ll be certain to feel more confident with that extra girth both in the locker room at Woodfield Country Club or Broken Sound and in the bedroom.

And that’s where modern medicine has quietly made some serious progress.

Modern Penis Enlargement: Why Girth Enhancement Leads

Forget the old-school stuff.

Pills? Marketing.
Pumps? Temporary.
Surgery? Overkill for most guys.

Today, the conversation is about precision and control.

At Boca Raton Men’s Clinic, the focus is on non-surgical girth enhancement, primarily using:

Why These Options Are Different (And Actually Work)

Here’s where it gets a little more technical…but not med school technical.

Unlike pills or devices, these fillers are:

  • Biocompatible – so they’re designed to work with your body, not against it
  • Structurally supportive  – they physically increase girth in a controlled way
  • Long-lasting – especially Bellafill girth enhancement, which stimulates your own collagen over time

Bellafill, in particular, is interesting because it’s not just a filler…it’s a collagen-building platform, meaning results can stabilize and even improve as your body responds.

HA fillers, on the other hand, offer:

  • Immediate results
  • Adjustability
  • A slightly softer, more flexible feel

Both have their place depending on your goals. Your vascular surgeon or other physician can chat with you about the differences and advantages of each.

A Quick Word on Safety (Because Girth Matters, But so Does Safety)

No legitimate medical procedure including girth enhancement is completely risk-free, and anyone telling you otherwise isn’t being honest.

With that said, when performed by an experienced practitioner (not a dermatologist, cosmetologist or med spa tech) these treatments are generally very well-tolerated.

Still, it’s important to understand:

  • Bellafill: In rare cases, small nodules or firmness can develop under the skin
  • HA fillers: Temporary swelling or uneven distribution can occur, especially early on

This is exactly why technique, experience, and product selection matter so much.

What Kind of Results Are Realistic?

This isn’t about unrealistic expectations.

Most men see:

  • A noticeable increase in girth
  • A more confident appearance
  • A meaningful difference in how things feel

And just as important – natural-looking results when done properly in a medical facility such as Boca Raton Men’s Clinic.

The Bottom Line

If you strip away all the noise, all the myths, all the locker-room talk…

Girth is what tends to move the needle.

Not because it sounds better.
Because it functions better.

And if you’re the type of guy who invests in improving things that matter…

This is simply one more area where knowledge—and the right approach—makes all the difference.
